Required Forms (all participants):
*Most required paperwork is now completed online during the registration process. Your student athlete MUST be registered to participate!*
REMINDER: Report cards are needed to determine academic eligibility. Please be sure to obtain a copy of your child’s report card as soon as they are published, as the school buildings have limited staff over the summer and may not be able to get you a copy until the start of the new school year.
